If you are current or former employees of Taconic whose W-2 data was compromised as a result of the Data Disclosure which occurred on or about January 30, 2017, you may be entitled to payment under the Settlement Agreement reached in the case. To recover as part of this Settlement, you must submit a Claim Form. Claim Forms must be submitted online or postmarked no later than July 15, 2020.

Documentation must be provided to support your claim as applicable. Documents should be clear, readable copies, as anything you submit will not be returned to you. If you are filing online, please have this information ready before you start to file, as your Claim will not be saved if you have to come back and finish at a later time. If you are unable to electronically upload a copy of your documents as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Claims Administrator. Please review the table below for the types of documentation you should submit to prove each Claim Category.
