Submit a Claim - Claim Instructions

If you are current or former employees of Taconic whose W-2 data was compromised as a result of the Data Disclosure which occurred on or about January 30, 2017, you may be entitled to payment under the Settlement Agreement reached in the case. To recover as part of this Settlement, you must submit a Claim Form. Claim Forms must be submitted online or postmarked no later than July 15, 2020.

You may submit claims in each applicable category from Claims A–E as described below:

  1. Payment for false/fraudulent tax returns filed after January 30, 2017;
  2. Payment for incidents of other identity theft related to financial fraud experienced after January 30, 2017;
  3. If you are claiming compensation under Claims A or B you may also claim compensation for time spent dealing with tax fraud or other identity theft;
  4. If you are not claiming compensation under Claims A or B, you may claim compensation for time spent in dealing with the ramifications of the Data Disclosure;
  5. Reimbursement for out-of-pocket expenses, other than purchase of identity theft protection service, as a result of the Data Disclosure.

Documentation Documentation must be provided to support your claim as applicable. Documents should be clear, readable copies, as anything you submit will not be returned to you. If you are filing online, please have this information ready before you start to file, as your Claim will not be saved if you have to come back and finish at a later time. If you are unable to electronically upload a copy of your documents as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Claims Administrator. Please review the table below for the types of documentation you should submit to prove each Claim Category.


Claim Category Supporting Documentation
Claim A: False/Fraudulent Tax Return Letters from the IRS, accountant, or another source showing that a fraudulent tax return has been filed.
Claim B: Other Identity Theft Related to Financial Fraud Receipts, voided checks, bank statements, or other documents showing the amount of your losses and/or a detailed narrative description.
Claim E: Out-of-Pocket Expenses Receipts, voided checks, bank statements, or other documents showing the amount of your losses and/or a detailed narrative description.

After clicking the button below, you will be required to provide the Unique ID you received with your Postcard Notice. If you did not receive a Postcard Notice or have lost yours, please contact the Claims Administrator to obtain your Unique ID. Then, you will be asked to provide your contact information, detailed information about the losses you incurred, and any documentation you wish to provide to prove those losses. Please have all your documentation ready as your Claim will not be saved if you have to come back and finish at a later time.
